Clomiphene citrate is a selective estrogen receptor modulator (SERM) primarily used in the treatment of female infertility due to anovulation, particularly in polycystic ovary syndrome (PCOS). It acts by competitively binding to estrogen receptors in the hypothalamus, blocking negative feedback from circulating estrogens. This leads to increased secretion of gonadotropin-releasing hormone (GnRH), which in turn stimulates the pituitary to release follicle-stimulating hormone (FSH) and luteinizing hormone (LH), promoting follicular development and ovulation. Clomiphene is administered orally, typically in 5-day cycles early in the menstrual cycle. It is also used off-label in men to treat secondary hypogonadism by increasing endogenous testosterone production through stimulation of the hypothalamic-pituitary-gonadal axis. Side effects may include mood changes, visual disturbances, hot flashes, ovarian enlargement, and, rarely, ovarian hyperstimulation syndrome (OHSS). Multiple pregnancy is a recognized risk.
