Mesterolone (commonly marketed as Proviron) is an orally active androgen and dihydrotestosterone (DHT) derivative used primarily in the management of male hypogonadism and certain forms of male infertility. Unlike anabolic steroids that primarily target muscle growth, mesterolone exhibits strong androgenic and weak anabolic properties. It does not aromatize into estrogen, making it useful in clinical contexts where estrogen suppression is desirable. Mesterolone enhances androgenic activity without significantly suppressing the hypothalamic–pituitary–gonadal axis at therapeutic doses. In fertility protocols, it is sometimes used to improve libido, sperm quality, and overall androgenic tone without impairing spermatogenesis. It binds with high affinity to androgen receptors and exhibits minimal hepatotoxicity due to the absence of 17α-alkylation. Side effects may include acne, increased aggression, hair loss in genetically predisposed individuals, and alterations in lipid profiles.
